Nutrition: Your Plate After 45
After 45, your needs change: more protein for muscle, calcium and vitamin D for your bones, fiber for the gut microbiome, and less room for extreme diets. This hub gathers what science really says about your plate at menopause.
What your plate can, and cannot, do
After forty-five, your needs change in measurable ways: more protein to limit muscle loss, a sufficient intake of calcium and vitamin D for bone, fiber for the gut microbiome. On the other hand, no way of eating "rebalances" your hormones, and very restrictive diets are especially counterproductive at this stage, because they worsen the loss of lean mass.
Protein: the most concrete lever
Muscle loss speeds up at the menopause transition: the body composition analysis in SWAN shows that lean mass declines while fat mass gain doubles in speed (PMID 30843880). A systematic review with meta-analysis concludes that a higher protein intake is associated with a benefit for bone health, particularly at the lumbar spine (PMID 28404575). In practice, that means spreading protein across all three meals, rather than concentrating it in the evening.
Calcium and vitamin D: useful, but not magic
This is an area where honesty is needed. In the large randomized trial of the Women's Health Initiative, calcium plus vitamin D supplementation produced only a modest effect on hip density and did not significantly reduce hip fractures in the general population studied (PMID 16481635). A randomized trial published in 2022 showed no reduction in fractures with a vitamin D supplement in middle-aged and older adults not selected for deficiency (PMID 35939577). The practical translation: correcting a deficiency makes sense; taking supplements "just in case" while expecting protection against fractures does not.
Weight: what a change in diet actually delivers
A randomized trial published in JAMA in 2022, involving 823 people of whom more than three quarters were women, compared a diet plus exercise program with simple follow-up: 7.7 kg lost versus 1.7 kg. For knee pain, the improvement was statistically significant but below the threshold considered clinically important (PMID 36511925). Losing weight helps; it does not fix everything, and saying so spares a lot of disappointment.
Supplements: clear-headedness is required
A meta-analysis of 69 studies covering twenty supplements concludes that glucosamine and chondroitin are "either ineffective or have small effects of questionable clinical importance". Turmeric and collagen show short-term effects, but with a quality of evidence the authors rate as very low, and no supplement keeps a clinically important effect over the medium or long term (PMID 29018060).
